Chiropractic Awareness Month Sweepstakes — Official Rules

1. Sponsor

SF Custom Chiropractic (“Sponsor”), 1365 4th St., San Francisco, CA 94158.

2. Eligibility

Open only to legal residents of California who are 18 years of age or older at time of entry. Further limited to residents whose primary residence is within a 10-mile radius of Sponsor’s clinic location listed above. Void where prohibited. Employees, contractors, and affiliates of Sponsor, and members of their immediate household, are not eligible.

3. Promotion Period

Begins 12:00:01 a.m. Pacific Time (“PT”) on October 8, 2025 and ends 11:59:59 p.m. PT on October 31, 2025 (the “Promotion Period”). Sponsor’s computer is the official time-keeping device.

4. How to Enter

(a) Online (Base Entry): During the Promotion Period, go to /october-sweepstakes (the “Promotion Site”) and submit the entry form with required fields (first name, last name, email, mobile phone, ZIP code) and the required checkbox acknowledging eligibility and agreement to these Official Rules and the Privacy Policy. Limit one (1) base entry per person.

(b) Alternative Method of Entry (AMOE) – Equal Dignity: On a 3″×5″ card, hand-print your full name, email, mobile phone, and ZIP code, place in a stamped #10 envelope, and mail to:

Must be postmarked by October 31, 2025 and received by November 4, 2025. Each properly completed mail-in counts as one (1) base entry. Limit one (1) base entry per person regardless of entry method.

Entry Limits/General: Automated, scripted, or bulk entries are prohibited and void. Multiple entries by the same person or email beyond the stated limits are void. In the event of a dispute, the authorized account holder of the email/phone submitted will be deemed the entrant.

5. Bonus Entry — Education Module (Optional)

During the Promotion Period, an entrant may earn one (1) bonus entry by viewing a short educational video on the Promotion Site (“Desk Check in 60 Seconds”) and submitting an answer to one quiz question (accuracy not required). Limit one (1) bonus entry per person, regardless of method.

AMOE for Bonus Entry (Equal Dignity): On a separate 3″×5″ card, hand-print your full name, email, mobile phone, ZIP code, and the words “Education Bonus”, and mail to the address above. Must be postmarked by October 31, 2025 and received by November 4, 2025. Counts as one (1) bonus entry. Online and AMOE bonus methods are mutually exclusive; max one (1) bonus entry total.

6. Prize / Approximate Retail Value (“ARV”)

One (1) Grand Prize: A year of musculoskeletal (“MSK”) care consisting of twenty-four (24) chiropractic treatments (exam, muscle work, adjustments, exercises) and twelve (12) 60-minute deep-tissue massages. ARV: $5,800. Appointments subject to availability and Sponsor’s standard patient policies. No cash alternative. Prize is not transferable and may not be substituted, except Sponsor reserves the right to substitute an item of comparable value if any portion of the prize becomes unavailable.

7. Winner Selection / Odds

Winner will be selected in a random drawing conducted by Sponsor on or about November 4, 2025, from among all eligible entries received. Odds depend on the number of eligible entries received.

8. Notification / Verification / Forfeiture

Potential winner will be notified by email and/or phone on or about November 5, 2025, and must respond within 48 hours of notification. Potential winner must complete and return any required documentation, including a W-9, and must schedule the first visit within 14 days of notification (on or before November 19, 2025). Failure to timely respond or complete required steps may result in forfeiture and selection of an alternate.

9. Conditions of Redemption / Patient Care

All services will be provided only after standard clinical screening and if clinically appropriate in the professional judgment of Sponsor’s providers. Missed/late-cancelled appointments may be counted as redeemed per clinic policy. Prize must be fully used within 12 months of the first visit; any unused services will be forfeited. Prize services cannot be transferred, assigned, or credited toward other services or products.

10. Taxes

Winner is responsible for all taxes and any costs not expressly stated herein. Sponsor may issue an IRS Form 1099-MISC (Box 3) based on the ARV. Winner must provide a completed W-9 as a condition of award.

11. Publicity

With winner’s consent, Sponsor may use winner’s first name/initial and city for promotional purposes in any media, without additional compensation, where lawful.

12. Privacy

Information collected is subject to Sponsor’s Privacy Policy available at the Promotion Site and will be used to administer the promotion and, if the entrant provides consent, for marketing communications. Entrants may opt out at any time.

13. SMS / TCPA Notice (Optional Marketing)

SMS consent is optional and not a condition of entry or purchase. If you opt in on the Promotion Site, you agree to receive recurring marketing texts from Sponsor at the mobile number provided. 14. General Conditions / Release

By participating, entrants agree to be bound by these Official Rules and Sponsor’s decisions, which are final. Sponsor is not responsible for lost, late, misdirected, damaged, garbled, or incomplete entries; technical errors; or any injury or damage to persons or property arising from participation or prize use. Sponsor reserves the right to disqualify any individual tampering with the entry process or acting in violation of these Rules, and to cancel, modify, or suspend the promotion if fraud, technical failures, or any other factor impairs the integrity of the promotion.

15. Governing Law / Disputes

All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, or the rights and obligations of entrant and Sponsor, shall be governed by the laws of the State of California, without regard to conflict of law principles. Venue shall lie exclusively in San Francisco County, California.

16. Winner List

For the winner’s first name/initial and city, send an email request to support@theleadingpractice.com with subject line “Winner List – October Sweepstakes” after November 12, 2025, and before January 31, 2026.
